Pancreatic and biliary tract cancer rapidly progress and the prognosis is extremely poor. It is necessary to manage patients with these cancers appropriately through diagnosis and treatments. Therefore, collaboration between oncologists and endoscopists is getting more important in these days. Although pathological confirmation is necessary at the final diagnosis, it is often difficult to get specimens in these cancers. In our hospital, the first choice is EUS-FNA or biopsy using ERC in locally advanced pancreatic and biliary tract cancer, and a system to refer patients who need endoscopic examinations from oncology to gastroenterology division has been established. Chemotherapy is basically performed in oncology division, and clinical trials are the first priority. It is important to share information of ongoing trials and standard treatments between two divisions through regular conferences. Patients with pancreatic and biliary tract cancer often develop obstructive jaundice and/or cholangitis. Biliary drainage using endoscopic approach is the first choice in our group. Sixty patients with pancreatic and biliary tract cancer underwent endoscopic biliary drainage in 2011, and 42 (70%) patients were treated in both divisions of oncology and gastroenterology. Thus, closed collaboration between oncologists and endoscopists is necessary to achieve good results in treatments for advanced pancreatic and biliary tract cancers and it would provide high quality medical service for patients with these. |
