Aims: To elucidate the efficacy of systemic chemotherapy(SC) for patients with unresectable pancratobiliary cancer (UPBC) and the prognosis of those patients treated by endoscopic intervention. Subjects: Three hundred and fifteen patients with UPBC treated by SC. Results: The proportion of EBD in patients with pancreatic cancer(PC) and patients with biliary tract cancer(BTC) was 41% and 65%, respectively. Since 2007, those patients with difficult EBD or re-intervention after stent obstruction were treated with ESBD. Of the 11 patients undergoing ESBD, 8 were PC and 3 were BTC. Of these 8 patients with PC, 5 were able to start or resume SC and their mean OS after ESBD was 5.3 months. All the 3 patients with BTC were able to start or resume SC and their mean OS after ESBD was 8.6 months. For patients with duodenal stenosis, duodenal stenting(DS) has been performed since 2006. Of the subjects, DS was performed for 25 patients. Nineteen patients suffered from duodenal stenosis after failure of SC and DS was performed as a palliative treatment (mean OS after duodenal stenting: 2.1 months). The other 6 with duodenal stenosis at initial examination or after failure of primary SC, were able to start or resume SC after DS(mean OS after DS: 6.6 months). Conclusions: Endoscopic intervention plays a significant role for the performance of SC in patients with UPBC. Even for the patients with difficult EBD or duodenal stenosis, endoscopic interventions such as ESBD and DS are effective for the introduction or continuation of SC, resulting in better prognosis. |
