Usefulness of WallFlex Partially Covered Stent in Patients with Unresectable Malignant Distal Biliary Obstruction

抄録 Background and aims: In chemotherapy for inoperable patients with obstructive jaundice, drainage is considered essential. WallFlex biliary partially covered stent(WF), which is improvement from Wallstent and has higher flexibility. In this study, we have prospectively examined the clinical usefulness of WF. Methods: The subjects were 48 patients with unresectable malignant distal biliary obstruction who underwent EST and were stented with WF in our hospital from April 2009 to October 2011. Patients were 23 males and 25 females aged 70.2 years on average. The diseases were pancreatic cancer in 42, bile duct cancer in 2, metastatic biliary obstruction in 2, IPMC in 1, and papilla cancer in 1 patient. Success rate of WF insertion, occurrence rate of procedural accident, occlusion rate of stent placement, patency period, and survival period were examined. Procedural accident which occurs within 30 days is regarded as early procedural accident, and the state which needed endoscopic treatment due to cholangitis or others was defined as occlusion.Results: Insertion of WF was successful in all patients, and it was possible to perform reduction in jaundice. There were two (4.2%) early procedural accidents, and mild pancreatitis and migration were found respectively in one patient. Occlusion rate was 10.4% (5/48), and the occlusion cause was biliary sludge in 3 and migration in 2 patients. Mean patency period of stent was 454 days, and median of the survival period was 179 days (49-915).Conclusion:WF is the stent with less procedural accident due to insertion, low occlusion rate, and long patency period.
