抄録 Background and aims: The management of jaundice and cholangitis is a key point to continue the effective chemotherapy for the patients with hilar biliary strictures due to unresectable biliary tract cancer (BTC). We performed endoscopic multiple deployment of metallic stents (MS) on these patients. We evaluated the efficacy of the chemotherapy to the patients receiving endoscopic multiple MS deployment due to BTC. Methods: Between March 2003 and December 2011, 69 consecutive patients with BTC received chemotherapy with gemcitabine, S-1 or gemcitabine with cisplatin. They received endoscopic multiple deployment of MS before or during chemotherapy. We inserted 7Fr plastic stents in 2 or 3 lobes for MS obstruction. We investigated anti-tumor effect, survival, MS patency, and the outcomes of MS obstruction. Results: The overall median survival time (MST) was 10.2 months, and overall median patency time was 4.6 months. The disease control (DC) rate was 64% (44/69). The MST was 16.3 months in DC group vs. 4.2 months in progressing disease (PD) group (P<0.01). The median patency time was 12.0 months in DC group vs. 3.2 months in PD group (P<0.01). Twenty-seven patients in DC group (61%) had MS obstruction. Among them, 24 patients (86%) could receive chemotherapy again after plastic stents deployment. The median time from stent obstruction to resumption of the chemotherapy was 28 days. Conclusions: Endoscopic multiple deployment of MS is effective for the chemotherapy to the patients with hilar biliary strictures due to unresectable BTC.
