Gemcitabine and Cisplatin combination therapy for biliary tract cancer with biliary drainage

抄録 (Background) In the chemotherapy for biliary tract cancer, cholangitis is considered a very important problem. Recently, gemcitabine and cisplatin combination therapy (GC) has become the standard therapy based on ABC-02 and BT22 trials. In these trials, GC was tolerable and cholangitis were manageable. In this study, the frequency of bone marrow suppression and cholangitis were investigated at Kanagawa Cancer Center. At our institution, oncologists perform biliary drainage with endoscopy.(Methods) The subjects were 41 patients treated with GC from 2010 to 2011. The primary sites were follows: gallbladder (11 patients), intrahepatic bile duct (20 patients), extrahepatic bile duct (9 patients) and ampulla of vater (1 patients). Before chemotherapy biliary drainage was done in 12 patients, and anastomosis between bile duct and intestine in 2 patients. GC was the 1st line in 27 patients, and the 2nd or 3rd line in 14 patients. GC was administered according to the standard schedule and doses of previous trials.(Results) The response rate was 15%, and the median overall survival was 13.5 months (limited to 1st line). Grade 3-4 leucopenia occurred in 29% of cases and neutropenia in 54%. On the other hand, grade 3 cholangitis was 7%. Bone marrow suppression was not always associated with cholangitis, and cholangitis could be treated with biliary drainage in 3.The frequency of cholangitis seemed not to be different between 1st line and others. There were no withdrawal cases due to cholangitis.(Conclusion) GC chemotherapy for biliary tract cancer can be performed well with biliary drainage in our system.
