＜Aim＞For MBPD, we studied roles of endoscopists when oncologists would start and continue treatment.＜Method＞A role of endoscopists is to give definite diagnosis if MBPD were suspected in CT. For such occasions, we perform EUS-FNA and give oncologists definite diagnosis and suggest treatment plans. We studied diagnostic rate of EUS-FNA in pancreatic cancer. Another role is to get rid of bile duct stricture(BDS) and gastric outlet obstruction(GOO) that may occur when oncologists continue chemotherapy. We studied the relationship between getting rid of them and continuation of chemotherapy, increasing survival.＜Result＞From 2007 to 2011, About cases of pancreatic cancer suspected, we performed 107 EUS-FNA. Class IV, V were 81 cases. The sensitivity of FNA about class IV, V was 75.7%. When we studied 20 cases with class IV, V who had surgery, we recognized agreement in 16 cases. We got results that sensitivity was 80%, specificity was 100%. About study of GOO caused by MBPD from 1996 to 2011, we performed 37 enteral stents. 36 cases improved symptom, and 5 of 14 cases could continue chemotherapy. About study of BDS caused by MBPD from 1996 to 2011, we performed 171 biliary stent. We got results that 170 cases could continue chemotherapy and get prolongation of survival.＜Conclusion＞We showed EUS-FNA made a contribution to diagnosis and therapy, and getting rid of BDS and GOO could make continuing chemotherapy and increasing survival for oncologists. We think a point that cooperation between endoscopists and oncologists make patients to keep their PS and continue chemotherapy. |
