Introduction: Chiba Cancer Center biliary tract and pancreas group consists of four physicians who specialized in both oncologic and endoscopic management for biliary tract and pancreatic cancer. In our institution, the attending physician of a patient performs oncologic management (e.g. anti-cancer treatment) as well as endoscopic diagnosis (e.g. EUS FNA) and therapeutic endoscopy (e.g. biliary stenting). Therefore, in case of emergency such as acute cholangitis, patients can receive therapeutic endoscopy immediately.Methods: We report our recent clinical data on anti-cancer treatment and therapeutic endoscopy in patients with unresectable pancreatic cancer (UPC).Results: From 2006 to 2011, 347 patients with UPC have received anti-cancer treatment in Chiba Cancer Center. Most of the patients had histologically or cytologically confirmed adenocarcinoma diagnosed mainly by EUS FNA. 307 patients with metastatic or locally advanced pancreatic cancer have been received gemcitabine based chemotherapy with a median survival of 9.1 months. 40 patients with locally advanced pancreatic cancer have been treated with chemoradiotherapy using S-1 with a median survival of 16.8 months. For biliary obstruction, expandable metallic stent was placed in 51 patients with a median patency of 11.8 months. 10 patients received gastrointestinal stent placement for gastrointestinal obstruction due to tumor invasion with a median patency of 2 months.Conclusions: To raise the quality of practice in patients with UPC, it is important for physicians to have enough knowledge of anti-cancer treatment and therapeutic endoscopy. |
