Non－steroidal anti－inflammatory drugs （NSAIDs） are widely used for their analgesicantipyreticandanti－inflammatory propertiesand aspirin is increasingly employed in the primary and secondary preven－tion of cardiovascular disease and ischemic stroke． Despite undisputed therapeutic efficacy for these indi－cationsa11 NSAIDs impart a considerable risk of peptic ulcer disease and upper gastrointestinal hemor－rhage． A growing body of evidence supports an association between non－aspirin NSAIDs and acute coro－nary syndromesand an expanding understanding of the gastroduodenal effects of aspirinCOX－2 selec－tive agentsclopidogreland H． pylori synergism fuel controversies in NSAID use． ln this reviewriskstratification of patients taking NSAIDs and the appropriate application of proven gastroprotective strate－gies to ・decrease the incidence of gastrointestinal hemorrhage based upon an individualized assessment ofrisk for potential toxicities will be discussed． Prevention of NSAID－related gastropathy is an importantclinical issueand therapeutic strategies for both the primary and secondary prevention of adverse eventsare continually evolving． |
