Background : Nonsteroidal anti-inflammatory drugs (NSAIDs) are widely used for many conditions in-cluding arthritis. lt is well-known that NSAIDs induce peptic ulcers in the stomach and duodenum. How-everthe effects of NSAIDs on the distal small intestine have been less clarified because of the difficulty ofendoscopic observation in the distal small intestine. Double balloon endoscopy (DBE) allows for the obser-vation of entire small intestine. The aim of this study was to evaluate the clinical features of the NSAID-induced small intestinal lesions using the DBEMethods : We have analyzed 15 consecutive patients with NSAIDs induced small intestinal lesionsamong 448 patients (911 cases) that have undergone DBE between September 2000 and March 2007 atJichi medical university hospital in Japan. The average age was 68.0 yr (range40-88 yr) ; 7 men and 8women. Thirteen patients had OGIBone patient had frequent vomiting and hypoalbuminemiaand onepatient had acute cholangitis.Results : Ulcerative lesions were observed in the ileum in 11 patients and in the jejunum in 2 patients.These patients were recovered uneventfully and had full resolution of the OGIB after the suspension ofthe drugs. Diaphragm diseases were diagnosed in the jejunum in 2 patientsand they were treated withendoscopic balloon dilations. Their symptoms disappeared after the procedures and had not recurredthereafter.Conclusions : NSAIDs can induce ulcerative lesions in the sma11 intestine even in patients with no ulcera-tive lesion in both ther stomach and colon. Endoscopic balloon dilation therapy is feasible for diaphragmdiseases. |
