The aim of this presentation is to discuss and clarify the merits and limits of NSAIDs including COX－2 in－hibitors for colorectal cancer prevention The suppressive effect of NSAIDs on colorectal tumors has beenin recognition since as early as 1981． This presentation will discuss theoretical and scientific bases of theeffect of NSAIDs on colorectal adenoma first． Then we w皿present our results of a randomized doubleblind clinical test to examine the regressive effect of a COX－2 specific inhibitor on adenoma of familial ade－nomatous polyposis comparing with other trials． Clinical guidelines of the use of aspirinNSAIDs andCOX－2 inhibitors for primary prevention of colorectal cancer will be introducedThe guidelines are pre－pared for the U．S Preventive Services Task Force． These results suggest that COX－2 inhibitors have a re－gressive effect to adenoma of the colon and rectum if a high clinical dose that may associate with a consid－erable degree of cardiovascular and gastrointestinal mucosal damage is used． We concluded that the re－gressive effect of NSAIDs and COX－2 inhibitor to adenoma of the colon and rectum may be introduced byharm to proliferative stem cells of the gastrointestinal mucosa． |
