抄録 Primary liver cancer95% of which is hepatocellular carcinoma (HCC)has ranked third in men and fifthin women as a cause of death from malignant neoplasm in Japan. The numbers of deaths and death ratefrom HCC showed a sharp increase beginning in 1975. Although the numbers of deaths (34510) and deathrates (27.4/100000) from HCC increased until 2004 in Japanannual deaths (33662) and the death rate(26.7/100000) from liver cancer decreased in 2006. Although both hepatitis B virus (HBV) and hepatitisC virus (HCV) are important causesHCV-related HCC represents 75% of all HCC in Japan. The incidenceof HCC without hepatitis B surface antigen (HBsAg) or anti-HCV accounted for 7-120/o of HCC in Japanand half of non-B non-C-HCC was of unknown origin. GeographicallyHCC is more frequent in westernthan eastern Japanand the death rates from HCC in each prefecture correlate with the prevalence ofanti-HCVbut not with HBsAg prevalence. lnterferon therapy for chronic hepatitis C has reduced therisk for development of HCCespecially among patients with sustained virological response. Further re-search should focus on mechanisms of carcinogenesis by HCV and HBVdevelopment of more effectivetreatmentsand establishment of early detection and treatment approaches. Better understanding of HCCunrelated to HCV and HBV and possibly because of steatohepatitis and diabetes should also be a majorconcern in future studies.
