抄録 Remarkable advances in diagnostic imaging modalities and the establishment of follow-up system ofhigh-risk population for hepatocellulart carcinoma(HCC)have made early detection of HCC possible. Ac-cording to the accumulation of the clinical and pathologic details of small HCCs of the early stagehumanhepatocarcinogenesis pathway has been documented in the past decade. Namelyit is evident that manyHCCs develope through the progressive pathway from pre-malignant nodular lesions (dysplastic nodules)to HCC in cirrhotic liver. ln the liverseveral cell types have the longevity which is needed to be the cell oforigin of a cancer : hepatocytescholangiocytes and progenitor cells. The latter are located in the most pe-ripheral branches of the biliary treethe ductules and canals of Hering. The most important risk factorsfor liver cancer are chronic viral hepatitis B and C and alcoholic and non-alcoholic steatohepatitis. ln theseand other chronic liver diseases progenitor cell activation is seenrendering them a target cell populationfor carcinogenesis. The degree of activation is positively correlated with the inflammatory activity and thestage of the disease. Recently it has been shown that in the cirrhotic stage of most chronic liver diseasesthe hepatocytes become senescent due to telomere shortening. This makes it even more plausible that atleast part of the hepatocellular carcinomas originate from a progenitor cellMicroarray analysisimmuno-histochemical phenotyping and correlation with clinical behavior confirmed this hypothesis. That is thereason why an international consensus group proposes a new classification using immunohistochemistryfor keratin 19as an early hepatoblast/progenitor cell marker. Several studies have shown that keratin 19is an independent prognostic factor and a marker for the progenitor cell origin of the cancer. The newclassification will be explained in detail
