Current Progress of Gastroenterology Issues in ICD-11 Revision

抄録 The progress in our understanding the mechanisms underlying gastrointestinal diseases is relentless andis followed by change in management. This progress is no longer reflected by the ICD-10 and thus WHOhas embarked in the new ICD-11 under the leadership of the J apanese Society of Gastroenterology.Diseases of the digestive system encompass a large spectrum including inflammatoryneoplasticmeta-bolic and neurovisceral entities. Same etiologies share diverse outcomes due to the complex interactionwith other factors (i.e. H. pylori gastritispeptic ulcergastric cancer)diseases may be organ limite d (i.e.esophagus) or spread systemically (i.e. M. Crohn)or interfere with other medical territories in the currentICD-10 (i.e. endocrinologyinfectiologyoncology).All these aspects have to be respected in the new revision of ICD (ICD-11).The Gastrointestinal Working Group (GI-WG) embarking in this challenge inciudes speciali. sts respectingglobal representation and the Japanese GI task force.The steps undertaken so far have been several preparatory meetings by the Japanese GI task forceGI-WG meetingsas well as face-to-face with chair members of the other territories of lnternal Medicine(IM-TAG).The advances in the complex process of reorganizing the code for the classification of gastrointestinal dis-eases (K-code) to this point includea) the updated definition of medical terminologiesb) the organ oriented listing of diseases andc) the introduction of etiology based classification of GI disorders.The new ICD-11 intends to be consistentrespects each individual organ as first category (Le. diseases ofesophagusstomach)includes all individual disease manifestations (second layer) and refers to etiology(third layer). Functional gastrointestinal disorders have been set up in a new category.The next stepsof the GI-WG will be : work on short definitions for each disease condition and find solu-tions for the classification of conditions overlapping several other medical territories.
