The progress in the management of H. pylori infection and related clinical conditions is relentless. Thespectrum of complications includes benign and malignant gastroduodenal pathologies and is related totopographic expression and severity of chronic gastritis. A series of extradigestive diseases are associatedwith the H. pylori infectionbut the evidence to date shows no definite protective effect of H. pyloriagainst astimaatopy and obesity.Mechanisms involved in diverse clinical outcomes include bacterialhost and environmental factors andare an area of intensive research.Prevalence of H. pylori associated diseases vary in Clifferent geographical areas and this demands theadoption of selective strategies in the management of H. pylori・ infection.Strategies aiming at the prevention of gastric malignancies have shown to be effective and even cost ef-fective in high risk populations.Eradication therapies find their main challenge is the worldwide observed increase of resistant H. pyloristratns.Options for first and second line treatme/nt regimen therefore need to respect the prevalence of antibioticresistance pattern. ln Europe we recommend to start with a clarithromycinamoxicillinPPI standard tri-ple regi’men only in areas with clarithromycin resistance〈200/o an/d to $witch to bismuth containing quad-ruple therapies in areas with clarithromycin resistance’2200/o. lf bismuth is not available sequential andnon bismuth quadruple therapies (concomitant therapy) should be used.Rescue therapies after failure have to be selected by respecting the antibiotic susceptibility in various geo-graphical regions and Qn individual susceptiibility testing.After 30 years since the diseovery of II. pylori we have come a long way with significant progress in theclinical management due to successful basictranslational and clinical researchbut the ultimate challengeof an H. pylori free healthy stomach is still in front of us |
