抄録 Introduction:Pancreaticobiliary maljunction(PBMJ)is well known to be an important risk of biliary tract cancer. We herein report characteristics of biliary tract cancer in patients with PBMJ as 2nd report of Japan-nationwide survey.
Patients and Methods:From 1990 to 2007, 2,529 patients with PBM were registered at 141 medical institutions in Japan. Clinical features in patients with biliary cancer were investigated in detail, according to presence of bliary dilataion(BD), time era, and age(pediatric or adult).
Results(Table 1):The overall incidences of associated biliary tract cancers were 11.1% in patients with BD and 37.5% in patients without BD. In pediatric patients, only one case with BD(0.1%)had a bile duct cancer. On the other hand, in adults, cancer incidence rose to 21.6% in those with BD and 42.4% in those without BD. The ratio of bile duct and gallbladder cancers in patients with BD were 32.1% and 62.3%, and in patients without BD were 7.3% and 88.1%, respectively. In patients with BD, cancer incidence in the latter period(2000 to 2007)was higher than that in the former period(1990 to 1999). The ratio of bile duct cancer increased in the latter period(from 5.5% to 9.3%). In patients without associated biliary cancers, cholecystectomy combined with the extrahepatic bile duct resection was performed in 82.1% of adults with BD, while, in only 29.8% adults without BD.
Conclusions:This nation-wide survey of PBMJ revealed characteristics of associated biliary tract cancers, and could be widely used as a reference data for diagnosis and treatment of PBMJ.
