Serum Helicobacter pylori CagA Antibody Titer was Associated with Strong Inflammation in the Corpus in Japan

抄録 Background:Subjects infected with H. pylori containing cagA do not always induce serum CagA antibody. Our previous meta-analysis showed that serum CagA seropositivity was associated with gastric cancer even in East Asian countries. At this point, it remains unclear the factors affected for serum CagA antobidy titer. Methods:90 H. pylori positive patients with gastritis were included. The status of cagA was examined by polymerase chain reaction. Serum CagA antibody titer, pepsinogen(PG)I, and PGII were evaluated by ELISA. Histological scores were evaluated according to Update Sydney System. CagA expression was examined by Western blot. Results:Seroprevalence of CagA antibody was found in 74.4% whereas all case was cagA-positive. Interestingly, serum CagA antibody titer was significantly correlated with PGI and PGII levels(P=0.003 and 0.001, respectively). Serum CagA antibody titer was also significantly correlated with mucosal mononuclear cell infiltration in the corpus(P=0.03). Bacterial density was not related with CagA antibody titer. No difference of CagA expression level of the strains was found from the subjects with low PG/low CagA antibody titer and high PG/high CagA antibody titer. Conclusions:Subjects with higher serum CagA antibody titer can be considered as high risk population for the development of gastric cancer from the point of strong gastric inflammatory response even in Japan.
