抄録 Background:To improve the prognosis of pancreatic cancer patients, more accurate serum diagnostic methods which could discriminate cancer from chronic inflammation are required. Methods:Sera from pancreatic cancer patients(PC), healthy volunteers(HV), and chronic pancreatitis patients(CP)were collected at multiple institutions. The PC and HV were randomly allocated to the training or the validation set. All of the CP were included in the validation set. The subjects’serum metabolites were analyzed by gas chromatography mass spectrometry. The diagnostic model constructed via multiple logistic regression analysis in the training set was evaluated on the basis of its sensitivity and specificity. Additionally, the differences of serum metabolome in PC and CP were analyzed to realize their relationship. Results:In the training set study, which included 43 PC and 42 HV, the model possessed high sensitivity(86.0%)and specificity(88.1%)for PC. The utility of the model was confirmed in the validation set study, which included 42 PC, 41 HV, and 23 CP and furthermore, it was found to be more accurate at early PC(77.8%)and CP(82.6%)than CA19-9. Although serum levels of many metabolites in PC represented the same tendency of CP, some metabolites contributed to discriminate it. Conclusions:Our model possessed higher accuracy than conventional tumor markers at detecting the early pancreatic cancer patients in cohort including chronic pancreatitis patients.
