The aim of this study is to evaluate the usefulness of magnified endoscopy for the diagnosis of lateranl extension of superficial esophageal adenocarcinoma EAC.Patients and method. 46 EAC from 34 patients were treated by ESD from 2000 to 2012. All of the patients were examined by Magnified endoscopy GIFQ240Z or Q260Z, and the lateral extension was diagnosed by the difference of surface and vascular pattern. The average age was 66. LSBE and SSBE was 30 and 4. The tumor length and observation duration was 13mm and 1051 days. The number of I, IIa, IIb, IIc, III was 8, 19, 8, 11 and 0. The lateral extension was diagnosed by color, depression or elevation by WLI, and surface and vascular pattern by NBI magnified endoscopy.R0 resection. 43 of 46 EAC were resected by R0 ESD. IIb spreading. 8 lesions showed IIb in growth patern. And, 18 of remaining 36 EAC had IIb spreading. The mean length of IIb spreading was 5mm. Only 5 of 18 IIb spreading were diagnosed by WL. However, 16 of 18 were diagnosed by ME.41% of EAC extended under squamous cell epithelium. The average lengh of extension was 3.9mm. Conclusion. The diagnosis of lateral extensin of EAC was difficult by WLI. However, the observation by NBI maginifyed endoscopy is useful for the diagnosis of lateral extension of EAC. |
