Background: Endoscopic biliary stenting is a well-established palliative treatment for unresectable malignant biliary strictures. Plastic tube stents (PSs) and self-expandable metallic stents (SEMSs) are most commonly used in these applications. However, few studies have been conducted for hilar biliary strictures (HBSs). We aimed to compare the efficacy of PSs and SEMSs for unresectable HBSs.Methods: 60 patients were enrolled and prospectively randomized into the PS (7-Fr FLEXIMA) or SEMS (10mm JOSTENT SelfX) group. The primary endpoints were the 6-month patency rates and the 50% patency period. Results: The 6-month patency rate was significantly higher in the SEMS group than in the PS group (81% vs. 20%, respectively; p = 0.0012). The 50% patency period was significantly longer patency in the SEMS group than in the PS group (359 vs. 112 days, respectively; p = 0.0002). The mean number of reinterventions for stent occlusion was significantly lower in the SEMS group than in the PS group (0.63 vs. 1.80 times/patient, respectively; p = 0.0008). The overall total treatment cost was significantly lower in the SEMS group than in the PS group (p = 0.0222).Conclusions: SEMSs were associated with a longer patency than PSs in patients with unresectable HBS. In addition, SEMSs were more advantageous in reducing the number of reintervention sessions and the overall treatment cost. |
