OBJECTIVES: Endoscopic bilateral metallic stenting has been introduced as feasible and effective palliative modality in patients with inoperable malignant hilar biliary strictures (HBS). However, repetitive endoscopic revision of occluded bilateral metallic stents may be challenging. The aim of this study was to evaluate the feasibility and efficacy of repetitive endoscopic revision for malignant HBS previously managed by bilateral metallic stenting.METHODS: Total 6 patients (5 cholangiocarcinoma and one gallbladder cancer) who had previously managed by bilateral stent-in-stent placement with cross-wired metallic stents were required additional sessions after first endoscopic revision of occluded stents during follow up.RESULTS: Total 19 additional endoscopic revisions were performed. The median number of additional endoscopic revision for each patient was 2.5 (range 1-8). Bilateral revision was performed in 6 (31.6%) endoscopic sessions. Metallic stents and plastic stents were used in 7 (36.8%) and 12 (63.2%) endoscopic revisions, respectively. Clinical success rate of endoscopic revision was 78.9% (15/19).CONCLUSIONS: Repetitive endoscopic revision for malignant HBS previously managed by bilateral metallic stenting was feasible and effective. Cross-wired metallic stents for malignant HBS may facilitate repetitive endoscopic revision after stent occlusion. |
