Background: MR enterography (MRE) for evaluating both small and large bowel lesions has been developed in patients with Crohn's disease (CD). The aim of this study was to evaluate the usefulness of MRE by assessing the small and large bowel lesions of CD patients with clinical and serological remission. We also assessed whether MRE could detect any CD lesions in patients with secondary loss of responsiveness for biologics. Methods: Thirty-six patients with established CD were prospectively examined by newly developed MRC. Two gastroenterologists and two radiologists who were blinded to the results of another study evaluated endoscopy and MRE findings, respectively. Results: In lesions at terminal ileum, the sensitivities and specificities of the MREC for all CD lesions were 100% and 75%. MRE could detect any lesions and stenosis in 58% (11/17) and 50% (10/20) of the patients with clinical remission (CDAI<150). MRE also detected any CD lesions in 39% (7/18) of patients with serological remission (CRP<0.35mg/dL). Furthermore, any CD lesions, strictures, and fistulas were detected in 100% (12/12), 75% (9/12), 17% (2/12) of patients with secondary loss of responsiveness for biologics. Conclusions: MRE can detect CD lesions even in patients with clinical and serological remission. MRE is also useful for assessment of CD lesions and strictures in patients with secondary loss of responsiveness for biologics. |
