Several studies have reported the advantages of forward-viewing echoendoscope(FV-ES) in the therapeutic and diagnostic EUS-FNA. However, the difference in the diagnostic EUS-FNA of upper gastrointestinal submucosal tumor(SMT) using FV-ES and oblique-viewing echoendoscope(OV-ES) was not clear. Aim: We evaluated the usefulness of the FV-ES and compared the diagnostic yield of EUS-FNA using FV-ES with that using OV-ES. Methods: 16 consecutive patients with upper gastrointestinal SMTs underwent EUS-FNA using both echoendoscopes. The following points were compared; (1) the diagnostic yield; (2) the EUS image quality assessed by a five points scoring system. Results: The puncture sites for lesions were as follows: esophagus 3, stomach 11, duodenum 2. The mean diameter of these lesions was 27.0±11.9mm. (1) The diagnostic yields with the FV-ES and OV-ES were 62.5%(10/16) and 62.5%(10/16), respectively. The combined results were 87.5%(14/16). (2) The mean scores of the EUS image quality in each location were also similar between the FV-ES(3.8±0.8) and OV-ES(3.5±1.5), except for significantly improved image qualities of the cervical esophagus and esophageal stricture using the FV-ES. None of the 16 patients had any complications. Conclusion: FV-ES facilitates the puncture of lesions that were difficult to access with the OV-ES. |
