EUS-Guided Choledochoduodenostomy (EUS-CDS) for Malignant Lower Biliary Tract Obstruction

抄録 (Objective)We retrospectively evaluated the safety and the efficacy of EUS-CDS with malignant lower biliary tract obstruction.(Subjects) We performed EUS-CDS in 72patients with malignant lower biliary tract obstruction from 2003 to 2012. We placed plastic stent (PS) in 34 patients from 2003 to 2009. Covered Metal Stent (CMS) was placed in 36 patients from 2009 to 2012. (Methods)The EUS-CDS method involved EUS-guided placement of a stent between the extrahepatic bile duct and duodenal bulb, as described previously (AJG 2011). (Results)1. Overall Success rate for EUS-CDS was 97% (70/72). 2. Median time for the procedure was 28 min (range:10-75 min).3. Functional success rate of EUS-CDS was 100% (34/34) in the PS group, and 94% (34/36) in the CMS group.4. Bile peritonitis was seen in 21% (7/34) with PS, and 3% (1/36) with CMS.5. Stent occlusions were seen in 57% (31/54) of the PS group, and 20% (10/50) of the CMS group.6. 50% stent patency according to Kaplan-Meier analysis was 120 days in the PS group, and was 460days in the CMS group.(Conclusion)Both technical success rate and functional success rate were so high in EUS-CDS. We needed short time for this procedure. Using CMS may prevent the occurrence of peritonitis and prolong the stent patency. EUS-CDS with CMS may be safety and useful procedure.
