Study aim: This study aimed to evaluate the feasibility of a novel laser system when performing endoscopic submucosal dissection(ESD) of gastric epithelial neoplasia. Patients and methods: 10 patients were enrolled into this study at a single tertiary referral center. All patients were diagnosed as gastric adenoma (n = 5) or adenocarcinoma (n = 5). A thulium 2-um wavelength laser system was used for all ESD procedures including marking, mucosal incision and submucosal dissection. Instead of endoscopic knives, a prototype device (550-um flexible silica fiber attached with metallic sheath) was inserted through the working channel of the endoscope. The main outcome measure was technical success and complication rates. Results: In all patients, ESD was completed using only thulium laser without the need to change other endoscopic knives. The median time for total procedure was 49 minutes (range, 35-203 minutes). In 80% (n = 8/10) of patients, active bleeding was not observed during ESD. The final pathologic mapping resulted in low-grade dysplasia (n = 4), differentiated adenocarcinoma (n = 5), and signet ring cell carcinoma (n = 1). Curative resection was achieved in 90% (n = 9/10) of patients. There were no significant complications, such as delayed bleeding and perforation, in any patients. Conclusions: The thulium laser system is feasible in ESD of gastric epithelial neoplasia. |
