The molecular classification of pancreatic neoplasia

抄録  The exomes of all of the major tumor types that arise in the pancreas have been sequenced, and at the genetic level pancreatic cancer is now one of the best understood of all of the cancers. The challenge for the next decade is going to be to translate this genetic understanding to improved patient care.

 An integration of the genetic findings with gross and microscopic tumor pathology has led to a new molecular classification that doesn’t throw out the old pathology. Instead, gross and microscopic pathology play an equal role with cancer genetics.

 This new understanding of the molecular classification of pancreatic neoplasia has several immediate clinical implications. 1)It can help us understand why pancreatic cancer runs in some families. 2)It adds to our understanding of the small precursor lesions(pancreatic intraepithelial neoplasia-PanIN)that give rise to invasive pancreatic cancer. 3)It adds to our understanding of the larger precursor lesions(intraductal papillary mucinous neoplasms and mucinous cystic neoplasms)that give rise to invasive pancreatic cancer. 4)It helps us classify invasive pancreatic cancer into clinically and prognostically significant groups. 5)It provides an understanding of the time-line to progression and metastases.
