抄録 | It had been clarified that NSAIDs including aspirin(ASA)caused mucosal injury in the entire gastrointestinal(GI)tract, because the capsule endoscopy(CE)enabled observation of the small intestine. Although, use of proton pump inhibitor is effective in prevention of upper gastrointestinal injury, prophylaxis against the small and large intestines has not been established. The aim of this study is to investigate the association between NSAIDs and mucosal injury in the entire GI tract. This was a retrospective single-center study. Of the 111 cases which underwent CE, 32 cases having Obscure GI Bleeding(OGIB)or abdominal symptoms with use of NSAIDs and/or ASA were analyzed about patient background, upper gastrointestinal endoscopy(EGD), lower gastrointestinal endoscopy(CS)and CE findings. A total of 31 cases could be observed the entire GI tract including small intestine by CE(20 cases were ASA users, and 11 cases were non-ASA NSAIDs users), 24 cases(77.4%)used acid secretion inhibitor. Abnormal findings(erosion, denuded mucosa, bleeding spot)were more frequent in the small intestine(upper GI:25.8%, colorectal:9.7%, small intestine:64.5%). The rate of abnormal findings in non-ASA NSAIDs users was higher than that of ASA users. A half of non-ASA NSAIDs users with mucosal injury affected multiple organs and were likely to seen in cases having several NSAIDs or steroid in combination. Use of NSAIDs causes small bowel mucosal injury. The observation of the entire intestinal tract is recommended for whom using several NSAIDs or steroid in combination. |
