Esophageal mucosal injury and symptoms in elderly persons taking low dose aspirin

抄録 It is known the elderly persons are more likely than younger to have Esophageal mucosal injury(EI). We also know there are many elderly patients taking low-dose aspirin(LDA)to prevent atherosclerosis diseases. But it is not identified whether LDA affects prevalence and symptoms of EI or not. We investigated prevalence, severity and symptom of EI between patients of LDA users(Users)and non-LDA users(Control)in the elderly persons(over sixty-five). Among 3059 elderly patients who underwent upper gastrointestinal endoscopy and answered FSSG and SF8-QOL were analyzed. Prevalence of EI was compared between Users and Control. Among patients diagnosed to have EI, symptom and QOL was compared between Users and Control. FSSG items were classified into TS, RS and DS. SF8-QOL were classified into PCS and MCS. Number of Users and Control was 355 and 2704, respectively. Prevalence of EI in Users:Control was 9.3%(n=33):9.4%(n=253)(p<0.001). TS, RS and DS were Users:Control=5.0±5.7:7.0±6.8(p=0.11), 2.8±3.5:3.9±4.1(p=0.12)and 2.2±2.7:3.0±3.2(p=0.15). PCS and MCS were Users:Control = 44.8±12.5:48.8±6.8(p<0.01)and 50.8±11.4:49.5±7.5(p=0.38). Prevalence was similar in Users to Control. Regarding symptoms, there was no difference Users and Control. But QOL was significantly impaired in Users. These results indicated symptom based management was not appropriate in patients with esophageal mucosal injuries taking LDA.
